NOTE: Submitting a medication request does not guarantee that it can be filled.

To request a prescription refill or written prescription, please complete the form below.

Important Guidelines:

• Lead Time: Please allow 24 to 48 hours for our veterinary team to review your pet’s records, confirm dosage, and prepare your request.

• Exam Requirements: Your pet must have been examined by a vet at Karrinyup Small Animal Hospital within the last 6 months (or more recently, depending on their health condition).

• Urgent Requests: If your pet needs medication urgently, please call us directly on (08) 9447 4644 instead of using this form.

• Written Prescriptions: We can provide written prescriptions for human or online pharmacies (a prescription fee applies).

• Special Orders: If your requested medication is a non-stocked item, we will contact you prior to ordering to arrange advance payment.

What Happens Next?

1. Review: Our Team will review your pet’s medical history. If a re-examination, blood test, or further information is required before dispensing, we will get in touch.

2. Notification: Once approved and ready, we will send you an SMS.

3. Payment & Pick-Up: Payment is collected when you pick up the medication at the clinic.
